Heywood Healthcare is now offering outpatient palliative care consultations for adult patients with advanced, serious illness. Historically this has only been offered to our oncology patients, however now patients with other chronic medical illnesses are welcome. Palliative care may be provided as a one time consultation or on an ongoing basis.
How to Refer a Patient:
- Fax referral/order for palliative consultation, office note, and any other pertinent records to 978-630-6158.
- Please obtain insurance authorization/referral.
- Hematology/Oncology staff will then call the patient to schedule the appointment.
- Currently this service is only being offered on selective FRIDAY mornings at Heywood Hospital.
Please explain to patients and families:
- Description of what palliative care is (see below).
- The consultation is typically a longer visit, up to 1 hour. Please plan accordingly.
- If desired, may bring their spouse or a family member to accompany them.
- We may make recommendations for pain management, however prescribing for controlled substances, if any, would be done by PCP and at their discretion.
