Sport Physicals Offered at Murdock School-Based Health Center
If you are having difficulty obtaining your sport physical over the summer, we can help!
Your child must be enrolled at the Murdock School-Based Health Center by having a signed parental consent form on file. If your child is not already enrolled, you may sign them up on the day of the physical appointment, or by completing a Consent/Enrollment Form sent home in the 1st day of school packet.

- Insurance will be billed (No co-pay required)
- All Insurances will cover only one physical per year
- 1 YEAR AND 1 DAY - Parents will need to know date of last physical to be eligible
