It is estimated that 1 million people will be diagnosed with diabetes in the next year. This is why Heywood Hospital is committed to promoting good health for people with diabetes. If you meet any of these criteria you would be a good candidate for the program.
- You have been recently diagnosed with diabetes
- You have had diabetes for sometime but are looking to improve your health
- You are changing from diet and or oral meds to insulin
- You are looking for a reliable source of up-to-date information and resources to help you improve your self management

How the Program Works:
The Diabetes Self Management proram at Heywood Hospital has current certification with the American Diabetes Association and is committed to help you live a healthy life with diabetes. The program is based on a national model that is monitored by the American Diabetes Association to assure quality care.
Your primary physician or diabetes specialist can refer you to the program
After You've Been Referred
Initial Assessment:
You will meet individually with a Certified Diabetes Educator. At this meeting you and the educator will decide how the program can best meet your needs.
Educational Classes:
Classes are taught monthly and include:
- Meal planning
- Eating out
- Food shopping and label reading
- Blood glucose monitoring
- Sick day guidelines
- How to choose an exercise program
- Coping with stress
- Goal setting and problem solving
- Medications
DSME Professionals
At your first appointment you will meet with a Certified Diabetes Educator who will help you assess your educational needs and assist you in setting goals for self management. You will be attending classes led by our professional team: Certified Diabetes Educator, Registered Dietitian, Registered Pharmacist and Cardiac Rehab RN who will help you reach your goals for self management of your disease.
Program Fees
Most health insurance companies willl cover the cost of the program. This includes Medicare patients. Your insurance carrier may require a co-pay.
